Incident Report

This form is used to record and respond to incidents within the school community, ensuring a safe and supportive environment for all students, staff, and visitors. It is essential that all incidents are documented accurately and in a timely manner to facilitate appropriate action and compliance with the Victorian Child Safe Standards.

Guidelines for Completion:

  • Provide clear, factual details of the incident, including date, time, location, and those involved.
  • Select the most relevant incident category to assist with classification and response.
  • Describe any immediate actions taken, including first aid, security responses, or staff notifications.
  • If the incident relates to child safety, wellbeing, or a security concern, indicate this so it can be appropriately escalated.
  • Attach any supporting documents if relevant.

Once submitted, this form will trigger an email notification to Larry.Roadcap@ybr.vic.edu.au, where it will be reviewed, recorded, and escalated if required. All reports are handled in accordance with the Privacy Act 1988 (Cth) and mandatory reporting obligations under the Children, Youth and Families Act 2005 (Vic).
