Anonymous Child Safety Reporting Form

This form is provided to ensure that any concerns regarding child safety are reported in a confidential and anonymous manner. The information you provide will be treated with utmost seriousness and confidentiality, and will be handled in accordance with the Victorian Child Safe Standards and the school’s Child Safety policies.

If you suspect that a child, or any person is in immediate danger, or risk of harm, CALL 000 IMMEDIATELY.

Instructions for Reporting:

• Please provide as much detail as possible to assist in the investigation of your concern.
• You are not required to include personal details (e.g., name, position, etc.). This form can be submitted anonymously.
• Please consider submitting this concen to a school designated Child Safety Officer and not anonymously, as this may help us better protect a child
• If you are a child or young person, please consider asking a trusted adult to assist you in completing this form.
• If you are a Mandated Reporter (including a staff member of Yeshivah - Beth Rivkah Colleges), please do not use this form, as it does not fulfil your legal reporting obligations
